Understanding SAP Evaluations: A Key Step in DOT Return-to-Duty Success

SAP Evaluation

A SAP evaluation is a federally mandated assessment required after a DOT drug or alcohol evaluation. It must be completed by a professional who is qualified under 49 CFR Part 40 Subpart O – meaning they have specific training and credentials to assess substance use and recommend appropriate services.

The evaluation is part of a larger, structured return-to-duty (RTD) process, designed to ensure public safety while providing employees with an opportunity to recover and return to work.

What Happens During a SAP Evaluation?

The evaluation process involves two main components:

1.    Initial Assessment

  • The SAP conducts a comprehensive clinical interview, assessing the nature of the violation, the employee’s substance use history, mental health, and lifestyle
  • The SAP then determines whether the employee needs education, treatment, or both
  • The employee receives a written plan outlining required steps

2.    Follow-Up Evaluation

  • Once the employee has completed the recommended treatment or education, they must return to the same SAP for a final evaluation
  • The SAP assesses compliance and determines whether the employee is cleared to take a Return to Duty test
  • If approved, the SAP also outlines a Follow-Up Testing Plan (at least six random drug/alcohol tests over the next 12 months, potentially up to five years).
